Julia’s Butterfly Foundation provides assistance to terminally and chronically ill children,  with a concentration in the tri-state area (NY/NJ/CT). We pay directly to the provider and never to the patient, to avoid any tax liability for the family.

All applications must be completed in full and be returned via email to info@juliasbutterflyfoundation.org, fax or returned in the mail to:

The application process includes:
-    Completed Request For Assistance Form
-    Letters (on official letterhead) from an affiliated doctor or social worker explaining the medical condition
-    Evidence of family’s financial situation
-    Child’s photo and a signed photo release (requested, not required).
