Julia’s Butterfly Foundation provides assistance to terminally and chronically ill children in the tri-state area (NY/NJ/CT). We pay directly to the provider and never to the patient, to avoid any tax liability for the family.

The Board meets monthly to review completed applications and typically provides a response within 2 weeks.     The application process includes:
-    Completed Request For Assistance Form
-    Letters (on official letterhead) from an affiliated doctor or social worker explaining the medical condition
-    Evidence of family’s financial situation
-    Child’s photo and a signed photo release (requested, not required).

All applications must be completed in full and be returned via email to info@juliasbutterflyfoundation.org or returned in the mail to:
